Intermediate Primary Care
Designed for individuals with one to two chronic health conditions. This plan includes all the benefits from Acute Care plus the following:
- Biannual visits
- Biannual lab work
- Quarterly health maintenance visits
- Coordination of age appropriate screening measures
- Referral to appropriate specialists as needed
- Case manager
Advanced Primary Care
Designed for individuals with multiple chronic health conditions. This plan includes all the benefits from Acute Care plus the following:
- Quarterly visits
- Quarterly lab work
- Coordination of age appropriate screening measures
- Coordination of care with specialists
- Case manager
Intensive Care
Designed for individuals with declining health requiring frequent visits, home care, hospice, and frequent hospitalizations. This plan includes all the benefits from Acute Care plus the following:
- Visits every four to six weeks
- Quarterly lab work plus additional testing as needed
- Coordination of appropriate screening measures
- Coordination and monitoring of home care
- Coordination and monitoring of hospice care
